Why your annual physical missed it

The seven patterns we see in nearly every intake.

We've reviewed a lot of panels on men 35–55. The same patterns show up again and again. None of them get flagged on a standard physical. All of them are measurable, and all of them are addressable — if you actually look.

PATTERN 01

Low-normal testosterone

What it feels like

Flat motivation, softer recovery, libido drift, the gym stops 'working' the way it used to.

What the panel shows

Total T in the bottom third of the lab range, low free T, SHBG climbing, estradiol-sensitive higher than it should be. Annual physical: 'normal.' Optimization range: not even close.

PATTERN 02

Visceral fat creep

What it feels like

Same diet, same training, but the gut is harder, the face is fuller, jeans fit different.

What the panel shows

Waist:height crossing 0.5, ApoB elevated, triglycerides up, HDL down, fasting insulin trending. Often the first signal of metabolic drift — years before A1c moves.

PATTERN 03

Broken sleep architecture

What it feels like

Seven hours in bed, four hours of real sleep. Wake at 3:14 a.m. Wired by 9 p.m. Foggy by 2 p.m.

What the panel shows

Low deep sleep on the wearable, elevated resting HR, low HRV, cortisol that's off-rhythm. The downstream effect on hormones, recovery, and cognition is bigger than most men realize.

PATTERN 04

Cardiovascular risk hiding in 'normal' cholesterol

What it feels like

Nothing. That's the problem.

What the panel shows

LDL 'normal' but ApoB high. Lp(a) never checked. hsCRP elevated. This is the silent pattern — the one that quietly stacks risk for a decade before it announces itself.

PATTERN 05

Suboptimal thyroid

What it feels like

Cold hands, dry skin, energy that drops off a cliff at 3 p.m., stubborn body fat.

What the panel shows

TSH 'within range' but free T3, free T4, and reverse T3 tell a different story. Almost never run on a standard panel.

PATTERN 06

Iron, ferritin & B12 quietly underwater

What it feels like

Endurance gone. Brain fog. Heavy legs on the third set. Mistaken for 'just getting older.'

What the panel shows

Ferritin low-normal, B12 low-normal, sometimes folate. Easy to fix — once you actually measure it.

PATTERN 07

Chronic stress signature

What it feels like

Short fuse. Disrupted sleep. Crashed libido. Gut symptoms. Belly fat that won't move.

What the panel shows

Cortisol off-pattern, DHEA-S suppressed, HRV chronically low, blood pressure creeping. The most under-treated pattern in high-performing men.
